The Michigan Primary Care Initiative is a strategic plan to resolve the major system barriers that impede the delivery of preventive services and limit the optimal management of chronic disease (and other conditions) in primary care settings.
As a result of the Initiative, and through a collaborative partnership with primary care, public health, and the broader health care community, the Michigan Primary
Care Consortium was established. Their shared vision is that "every consumer of primary care services in Michigan is consistently receiving evidence-based preventive and chronic disease care, and changes to assure quality care are embedded into primary care practices across the state." |
