System Barriers Addressed
In September 2004, stakeholders identified five health system barriers as priority areas for improvement. These barriers were chosen because of their importance, impact and their potential for improvement:
- Limited patient access to non-primary care health professionals and community resources
- Limited access to comprehensive patient and population health information
- Payment systems and reimbursement structures that limit services
- Multiple guidelines for care
- Provider visits that emphasize episodic, acute care rather than continuous health promotion and disease management
Five workgroups were formed to develop specific goals, objectives, and strategies for addressing each of these barriers. Two co-chairs led each of the workgroups and members represented a variety of organizations throughout the state. |
